About The Position

The Home Visitor works within the Community Asthma Prevention Program (CAPP). CAPP seeks to educate families and to improve self-management skills in managing asthma and environmental mitigation in order to improve asthma control for patients. The Home Visitor's primary responsibility is to educate, and to provide health care and community resource connections in order to improve asthma management. Home visitors support families for one year after in-home or virtual educational encounters that occur both in-person and virtually. For video/virtual home visits the Home Visitor will educate patients and families in downloading, connecting and navigating virtual visits for appointments and resources in order to successfully navigate and connect families to community resources. The Home Visitor will contact the family at regular intervals and documents how the intervention is maintained in the home. The Home Visitor is also available to the family as needed between visits and can help to schedule visits in the clinic or via telehealth for CHOP patients. The home visitor is responsible for a minimum caseload and documentation of initial and follow-up visits in the electronic health record (EHR) and other electronic data capture systems.

Requirements

  • High School Diploma / GED Required
  • At least three (3) years working in community-based programs, community health Required
  • educating community residents
  • Ability to travel by car or independence on public transit, taxi, or ride-sharing program
  • Knowledge of local community agencies and organizations.
  • Strong interpersonal skills, communications skills, problem-solving skills, teaching and coaching skills, and community networking skills.
  • Ability to work effectively with a wide range of constituencies in a diverse community.
  • Excellent computer skills; email, Word, EPIC
  • Understands and values the importance of equitable access to healthcare
  • Ability to coach and navigate others in utilizing technology related to telehealth

Nice To Haves

  • Bachelor's Degree in social work, public health, education Preferred
  • Philadelphia resident Preferred
  • Personal experience with medical complexity preferred but not required
  • Outstanding customer service and communication skills
  • Ability to work independently and in collaboration with other teams
  • Ability to navigate Smartphones and mobile devices; IOS and Android

Responsibilities

  • Conduct initial assessment of home to evaluate for asthma triggers, conduct asthma education classes in the home, demonstrate and supervise environmental interventions in the home.
  • Assist patients and families in identifying education, information, agency referrals and advocacy to assist in managing medical and social needs.
  • Maintain caseload of 50 families.
  • Call to set up appointments with eligible community members for the home visitor program.
  • Follow all protocols
  • Connect families to community agencies, social service organizations, insurance providers and other resources according to identified needs through in-person or virtual assistance.
  • Conduct regular follow-up in-person or virtual visits to reinforce education and assess maintenance of intervention.
  • Document all visits, activities with family, supplies used, etc and enter data into database and electronic health record.
  • Responsible for appropriate use of incentives and intervention supplies, maintaining organization of storage closet and documentation of use of supplies and incentives
  • Assist in training new Home Visitor staff.
  • Attend all CAPP staff meetings.
