Provides in-home education, support and resource services to children, parents, pregnant women and families in accordance with their specific needs. This role involves collaborating with parents to develop and implement individualized programs, promoting parent-child bonding, maintaining case notes and records, assessing family needs, enhancing family functioning, ensuring full enrollment through recruitment, strengthening family knowledge of child development, assisting with household management, planning for continuity of care, providing community resources and referrals, conducting weekly home visits, and developing a working knowledge of ethnic and cultural differences of client families.
