About The Position

Glenwood is seeking a Home Visitation Coordinator to join their team. This role will oversee the intake and referral processes, coordinate family engagement activities, and support program operations for children and families impacted by autism spectrum disorder and related developmental disabilities. This position plays a critical role in ensuring high-quality, family-centered services through strong coordination, communication, and data management. This is a part-time position working 20 hours per week.

Requirements

  • Requires a bachelor's degree from an accredited college or university in psychology, social work, public health, education, neuroscience, human services, or a related field.
  • Experience coordinating programs, managing data systems, conducting intake processes, or supporting family-centered services is preferred.
  • Experience working with individuals with autism spectrum disorder and/or developmental disabilities is required.
  • Must be 21 years of age or older.
  • Valid Alabama driver’s license with insurability through the agency’s carrier.
  • Must clear DHR Child Abuse & Neglect Registry.
  • Must obtain certification in CPR, First Aid, and Managing Crisis Safety (MCS) within 30 days of employment.
  • Demonstrates knowledge of ethical standards, informed consent, confidentiality requirements, and best practices in services for individuals with autism spectrum disorder and developmental disabilities.
  • Provides effective leadership, guidance, and support to staff to promote consistent program operations and service quality.
  • Communicates clearly and professionally with families, staff, clinicians, community partners, and stakeholders.
  • Maintains strong organizational skills, attention to detail, and the ability to manage multiple priorities in a timely manner.
  • Contributes to a positive, respectful, and mission-driven work environment.

  • Uses basic computer systems, documentation tools, and communication platforms to complete reports, maintain records, enter data, and support program operations.

Responsibilities

  • Coordinate daily Home Visitation Program operations to ensure efficient service delivery, timely communication, and compliance with program requirements.
  • Manage intake, referrals, enrollment, and service initiation for participating individuals and families.
  • Support recruitment, engagement, and retention of individuals with autism spectrum disorder and related developmental disabilities and their families; serve as a key contact for participants, caregivers, and community partners.
  • Coordinate home visits, assessments, consultations, follow-ups, and related activities to support continuity of services.
  • Maintain required program documentation, intake records, consent forms, service logs, and related records in accordance with agency and regulatory standards.
  • Collect, enter, and monitor program data in approved systems, ensuring accuracy, confidentiality, and privacy compliance.
  • Assist with assigned assessments, interviews, questionnaires, and outcome measures as trained.
  • Collaborate with clinicians, therapists, staff, families, and community partners to integrate home visitation services with broader clinical and support systems.
  • Prepare reports and communicate program activities, outcomes, progress, and needs to leadership and stakeholders.
  • Support quality assurance, audits, monitoring, and continuous improvement while maintaining compliance with organizational, ethical, funding, and regulatory requirements.
  • Participate in ongoing training and remain current on best practices in autism services, developmental disabilities, family-centered care, ethics, and program coordination.
