Join UChicago Medicine as a Home Visit Nurse with Family Connect, an evidence-based program that connects parents of newborns to community resources through postpartum nurse home visits. Developed in partnership with Duke University, this community-based program supports new parents, offers physical assessments of mother and baby, addresses care questions, identifies parental needs, and facilitates referrals to community services. The program also aims to identify gaps in community resources to advocate for increased services. The Home Visiting Nurse will provide home visits to all families with newborns in Chicago, assessing needs across 12 domains of physical and psychosocial well-being and facilitating referrals. This role involves working within a team of clinical and non-clinical staff to support families using the Family Connects protocol. Additionally, the nurse will teach a minimum of one in-person prenatal education class per week, covering topics like pregnancy, newborn care, and breastfeeding, serving as a clinical resource for expectant and new families. Effective communication with patients, families, community partners, and interdisciplinary team members is essential.
