The Home Therapy Registered Nurse (RN) is an entry-level position within the Clinical Advancement Program (CAP). This role is responsible for providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. The RN assesses and manages patients' response to home dialysis training and treatment therapy, following predetermined protocols and communicating patient issues to the physician. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership, and quality improvement activities to enhance patient care outcomes and facility operations. The role involves empowering, engaging, and training dialysis patients on peritoneal and home hemodialysis using state-of-the-art programs and equipment. Primarily, the role functions in a clinic setting, conducting initial home visits with another staff member present, and serves as an onsite Patient Educator and Case Manager.
