About The Position

DaVita is seeking an organized, self-directed RN to manage Home Hemodialysis (HHD) patients. You’ll educate patients and caregivers in-clinic, enabling them to perform dialysis independently at home. This role blends clinical care, patient education, and case management. This hire will also cross-train in Peritoneal Dialysis (PD). Flexible Monday-Friday 8am-5pm type-of schedule. No Weekends, No Holidays! Will be covering census in one facility; but must be flexible to support our Westchester area Dialysis Centers as patient needs arise.

Requirements

• RN license in the state of practice
• Minimum of 12 months of RN experience
• Valid driver’s license, insurance, and reliable transportation
• ADN required
• Basic computer skills (MS Word, Outlook)

Nice To Haves

• Dialysis (PD, HHD, chronic, or acute)
• Patient education or teaching background
• Case management or telehealth experience
• CNN or CDN certification
• BSN preferred

Responsibilities

• Train patients and caregivers on HHD procedures and self-care
• Manage a caseload of HHD patients, including assessments and care planning
• Monitor treatment progress, conduct home visits, and provide on-call support
• Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes
