About The Position

The Home Service Worker provides non-medical personal care and supportive services to clients in their homes. This role assists clients with activities of daily living (ADLs), promotes independence, and ensures a safe, comfortable home environment while maintaining dignity and respect. Requirements

High school diploma or equivalent required.
Knowledge of basic personal care and homemaking services.
Understanding safety practices and infection control procedures.
Knowledge of client rights and confidentiality standards.
Ability to assist clients with activities of daily living safely and respectfully.
Strong communication and interpersonal skills.
Ability to follow care plans and instructions accurately.
Ability to recognize and report changes in client condition.
Ability to maintain professionalism, compassion, and respect for clients.
Ability to work independently in a client home setting.
Ability to work collaboratively with clinical and administrative teams.
Ability to read, write and speak English sufficient to perform job duties and to interact and communicate with residents, family members and employees.

Nice To Haves

Previous experience in home care, caregiving, or related field preferred.
Experience assisting with personal care and activities of daily living preferred.
Certification as a Personal Care Assistant (PCA) or Homemaker preferred, if required by state or agency policy.
Valid driver’s license and reliable transportation preferred (if transportation duties are required).

Responsibilities

Assist clients with activities of daily living (ADLs), including bathing, grooming, dressing, and toileting.
Help with mobility, transfers, and ambulation as directed in the care plan.
Assist with meal preparation, feeding, and maintaining proper nutrition and hydration.
Provide medication reminders as permitted by agency policy.
Monitor client condition and report changes to the supervisor promptly.
Provide companionship and emotional support to clients.
Assist with light housekeeping, laundry, and maintaining a clean living environment.
Assist with shopping, errands, and transportation assigned.
Support clients in maintaining independence and daily routines.
Maintain a safe environment and follow established safety procedures.
Observe and report any safety hazards, changes in client condition, or concerns to the supervisor.
Follow infection control practices and proper hygiene standards.
Respect client rights, privacy, and confidentiality always.
Document services provided according to agency policy and procedures.
Follow the individualized care plan and agency guidelines.
Maintain compliance with applicable state regulations, including standards established by the Illinois Department of Public Health (IDPH), when applicable.
Attend required training and in-service education sessions.
Comply with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all Plymouth Place policies and procedures.
Maintain regular and punctual attendance at work and meetings.
Must be flexible to work in all levels of living (Independent, Assisted, Memory Care and Skilled).
