Requirements

  • High school diploma or GED is preferred but not required.
  • Valid Tennessee, Kentucky, or Alabama driver’s license.
  • Ability to read and follow directions.
  • Ability to communicate effectively in writing.
  • Satisfactory criminal background check and clear driving record.
  • Ability/availability to attend and pass all training courses offered and required Department of Intellectual & Developmental Disabilities (DIDD) and PDI training.
  • 1-2 years experience working with individuals with intellectual and/or developmental disabilities.
  • Supervisory experience is preferred.
  • Successful completion of DIDD Core and PDI Training and the development of knowledge of persons supported.

Responsibilities

  • Ensures the health, safety, and well-being of the person(s) served residing in the home.
  • Provides instruction and/or assistance to individuals with disabilities with respect to improving independence in personal hygiene, self-care skills, money management, housekeeping, recreation, and promoting community involvement.
  • Directly supervises assigned staff on duty at residential sites during assigned hours and serves as the primary backup for Direct Support Staff.
  • Involved in the training and scheduling of staff to ensure properly trained staff members are present.
  • Assists staff on duty and persons served in all aspects of daily living activities and maintains a safe and healthful environment, including filling shifts as needed.
  • Attends all scheduled meetings and mandatory training.
  • Ensures the health, safety, and welfare of the persons served.
  • Provides training to direct support staff on documentation requirements.
  • Performs 90-day Performance Evaluations for new employees.
  • Completes and submits staffing schedules, with trained staff, two weeks in advance.
  • Ensures persons served have adequate staff coverage by trained support staff.
  • Reviews receipts for household, groceries, and personal funds, ensuring spending ledgers are available, current, and accurate.
  • Ensures persons served have access to their personal funds and oversees the daily accounting of funds.
  • Completes the Daily Home Manager Checklist while on shift to ensure daily oversight of Medical Administration Records (MAR), Spending Ledgers, Daily Notes, Communication Log, grocery supply, medication supply, environmental safety, vehicle safety, etc.
  • Reviews Staff Sign In/Sign Out Log to verify accuracy.
  • Reviews Travel Books to ensure information reflected in the Health Passport, Individual Service Plan (ISP), Daily Note, Community Based/Facility Based (CB/FB) Log, etc. is accurate.
  • Reviews Medical Administration Record (MAR) Book daily to ensure MAR sheets are consistent to the prescription label as well as the physician order.
  • Ensures compliance with Medication Variance Reporting requirements.
  • Ensures the individual is supported in the most respectful and professional manner.
  • Ensures fire drills are conducted monthly, on varied shifts, and submits appropriate documentation.
  • Ensures allegations of Abuse and Neglect are reported within a 4-hour time frame.
  • Accompany individuals on all scheduled or emergency medical appointments/consultations.
  • Provides staff coverage for the individuals, as needed and required.
  • Conducts and completes the Comprehensive Home Audit Tool weekly and reports findings.
  • Attends and participates in annual Individual Service Plan (ISP) and Circle of Support (COS) meetings.
  • Reviews Occupational Therapy (OT), Physical Therapy (PT), and Speech-Language Pathology (SLP) Assessments, Individual Service Plans (ISPs), and Behavior Support Plans (BSPs) on an ongoing basis.
  • Coordinates competency-based training for direct support staff by appropriate professionals.
  • Ensures daily implementation of Individual Service Plan (ISP) outcomes and therapy plans.
  • Ensures there is an updated copy of the Operations Manual in the home.
  • Maintains travel records in accordance with the Department of Intellectual & Developmental Disabilities (DIDD) operations manual.
  • Assumes primary responsibility for using Food Stamps/EBT cards, including including the individual in grocery shopping and household expenses.
  • Oversees daily accounting of personal, household, and grocery funds.
  • Is on call 24 hours a day, 7 days a week, with flexible scheduling.
  • Submits sign-in logs to the VP in a timely fashion.
  • Obtains authorization for overtime.
  • Manages employees' substandard performance and documents it in a counseling session.
  • Keeps Residential Managers and Program Coordinators informed of staffing issues, vacancies, and requests.
  • Keeps the Agency Chief of Nursing or designee, as well as the management team, abreast of medication-related issues and missed appointments.
