Home Infusion RN - Independent Contractor (Schenectady, NY)
About The Position
AmeriPharma is seeking a dedicated Registered Nurse with home infusion therapy expertise. The role combines infusion therapy with patient education, providing in-home care and support to patients receiving treatments such as TPN, IVIG, and IV biologics. The RN provides advice and education by telephone and in person, and offers interventions, counseling, and support requiring nursing knowledge and judgment across psychological, biological, physical, and social sciences. Under the supervision of the Director of Nursing, the RN makes decisions based on nursing education and experience and clinical guidelines to support compliance and persistence in home infusion programs. The position is per diem with flexible hours and is engaged as an independent contractor. The work includes patient visits in the home and telephone consultations.
Requirements
- Graduate of an accredited nursing school.
- Current RN state license.
- At least 2 years of experience in home infusion and infusion centers.
- At least 2 years of experience with IV immunoglobulins and IV biologics.
- Effective interpersonal, time-management, and organizational skills.
- Strong written and verbal communication skills.
- Ability to maintain consistently accurate records.
- Ability to direct information to the appropriate healthcare provider.
- Computer proficiency, including word processing and efficient use of the internet and email.
- Current CPR and BLS certification through the Red Cross and/or AHA.
- Valid driver’s license, reliable transportation, and automobile insurance.
Nice To Haves
- CRNI (Certified Registered Nurse Infusion) certification is preferred.
- Experience with peripheral IV starts, infusion therapy, and phlebotomy for adult and pediatric patients is preferred.
Responsibilities
- Conduct in-home infusion therapy visits according to each patient’s plan of care.
- Prepare clinical and nursing notes and consistently document patient interventions in clinical and non-clinical records.
- Serve as a clinical resource and collaborate with other pharmacy departments.
- Maintain knowledge of drug side effects and interactions.
- Provide telephone and in-person patient education about therapy protocols, drug side effects, and home infusion procedures.
- Ensure patients understand their therapy plans, medications, and the importance of adherence.
- Act as a liaison among patients, their families, and the pharmacy during initial and ongoing consultations.
- Perform comprehensive initial and periodic head-to-toe assessments and initiate preventative and rehabilitative care.
- Use assessment data and input from the agency team, physician, patient, and family to determine patient needs.
- Manage patient and family expectations regarding services, outcomes, and discharge goals.
- Establish primary and secondary diagnoses based on patient assessment and the home health care focus.
- Develop care plans with skilled interventions, medical supplies, and services needed to meet goals.
- Evaluate patient progress with team members and revise care plans as needed.
- Perform other duties as assigned.
