About The Position

The JHIS Inpatient Coordinator is primarily responsible to contribute to the interdisciplinary Team approach to assessing the eligibility/appropriateness of clients referred to our service for home infusion therapies. The Inpatient Coordinator will work with the client, MD and primary JHIS Pharmacist assigned to a client’s care to identify the safest and most effective plan of care for infusion services. Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson. Consults with other health professionals regarding infusion specialty specifics that directly or indirectly impact their clinical practice. Participates in patient care planning communication with the interdisciplinary team regarding the appropriateness of proposed therapies for individual clients. Presents information regarding the proposed therapies, rights/responsibilities and financial, considerations to clients and significant others prior to therapy initiation. Educates clients/caregivers regarding specific infusion related procedures so that independence of performance and safety is assured. Communicates with Home Care Case Coordinators, Hospital Case Managers, physicians, Home Health Agencies and interdisciplinary JHIS team regarding the disposition of clients prior to and immediately after discharge from the inpatient facility. Performs client education in an efficient and effective manner. Assesses the learner’s barriers to learning, the preferred learning style of the learner, strives to overcome barriers to education and supports the process of education to foster the learner’s independence in procedure performance. Performs associated duties as assigned by Nurse Manager or Director. Communicates verbally and in writing to physicians regarding the status of clients on home infusion therapy. Attends mandatory education programs as required.

Requirements

  • Graduate of accredited nursing school
  • 2-3 years combined experience in Home Care or Home Infusion Therapy environment
  • Current RN licensure in state of employment
  • Provider in Basic Cardiac Life Support
  • Valid driver’s license

Nice To Haves

  • Chemo certification recommended
  • Case management experience helpful
