Home Hospice RN Case Manager
About The Position
Community Hospice of Vidalia is currently searching for full-time RN CASE MANAGER to provide end-of-life care to home hospice patients in their last stages of life. Qualified RNs will be caring, compassionate, and patient and will enjoy providing education and care to both patients and families. Qualified RNs will communicate well with an interdisciplinary team and will be motivated and self-driven. Position requires travel up to 60 miles from Vidalia. Must have valid driver’s license and reliable transportation. Hours are Monday through Friday, 8:30am until 5:00pm with one weekend of on-call every six weeks. Community Hospice was established in 1999 and has been a pillar in our community since opening our doors. We provide quality, compassionate end-of-life care for patients in need of our services. Requirements
