About The Position

The Home Hospice Nursing Assistant (CNA) in Manitowoc County plays a vital role in providing compassionate, patient-centered care to individuals facing life-limiting illnesses in their homes. This position focuses on supporting patients' comfort and dignity by assisting with daily living activities, monitoring health status, and offering emotional support to both patients and their families. The CNA works closely with the interdisciplinary hospice team to ensure that care plans are effectively implemented and adjusted according to patient needs. This role requires a deep commitment to empathetic care, excellent communication skills, and the ability to work independently in a home setting. Ultimately, the CNA contributes significantly to enhancing the quality of life for patients during their end-of-life journey while respecting their wishes and cultural values.

Requirements

  • Current and valid Certified Nursing Assistant (CNA) certification in the state of Wisconsin.
  • High school diploma or equivalent.
  • Demonstrated experience in providing personal care to patients, preferably in a home health or hospice setting.
  • Strong communication skills and the ability to work independently in a home environment.
  • Valid driver’s license and reliable transportation to travel within Manitowoc County.
  • Ability to work four days per week with a rotating weekend schedule.

Responsibilities

  • Provide direct personal care to hospice patients, including bathing, grooming, dressing, and mobility assistance.
  • Monitor and document patients’ physical and emotional conditions, reporting any changes promptly to the nursing staff.
  • Assist with basic medical tasks such as taking vital signs and supporting medication administration under supervision.
  • Offer compassionate emotional support and companionship to patients and their families, helping to alleviate anxiety and promote comfort.
  • Collaborate with the hospice care team to implement individualized care plans and participate in care coordination activities.
  • Maintain a safe, clean, and comfortable environment in the patient’s home, adhering to infection control and safety protocols.
  • Educate family members on basic care techniques and provide guidance on how to support their loved ones effectively.
