About The Position

The Hospice RN Weekender reports to the Team Manager and is responsible for identifying patient/family care needs, and for providing nursing and supportive care for terminally ill patients and their families. This is a full-time Home Hospice Nurse position, working every Saturday and Sunday from 8 AM to 8 PM. GBMC HealthCare, situated in Towson, MD, is a healthcare provider with over 60 years of legacy, operating a suburban campus with over 250 beds. Specifically, it involves working with Gilchrist, Maryland's premier nonprofit provider for serious illness and end-of-life care for over three decades, and Hospice of Washington County (HWC) in Hagerstown, MD, the only licensed hospice provider in Washington County. The focus of hospice care is on caring for the individual, not the cure, helping them live life to the fullest and make self-determined decisions during their final season. Hospice care emphasizes quality of life and is provided wherever the individual designates as their home, including private residences, nursing homes, or assisted living facilities. It is available to patients of any age, religion, race, or illness and is covered by Medicare, Medicaid, and most private insurance plans.

Requirements

• Diploma, ADN and/or BSN from an accredited institution
• Current Maryland State license as a Registered Nurse
• One to two years of nursing.
• Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection
• Strong time management skills
• Excellent interpersonal and communication skills
• Ability to be patient and family focused
• Skill in the use of EMR (Electronic Medical Record) and/or ability to learn
• Skill in the use of Blackberry and portable computer

Nice To Haves

• Home Health or Hospice experience preferred.

Responsibilities

• Completes initial assessment of patient and family to ascertain home care needs - including physical, emotional, spiritual and mental status.
• Prepares and implements the care plan for patient/family unit in cooperation with the Interdisciplinary Group.
• Provides direct nursing care to patients in order to maintain highest level of care for the patient in conjunction with the primary physician.
• Cooperate with Clinical Team Leader in utilization of other team members, outside agencies, and community resources. Updates primary physician when necessary and regularly on a monthly basis. Updates interdisciplinary team at patient care conference
• Teaches family members or hired caregivers, patient care skills, including: pain control, medication (RN titrates medication within prescribed range), injections, draws blood for labs, manages IV's, dressing changes, hygiene, nutrition, checking vital signs in accordance with orders from physician, as well as addressing emotional needs.
• Ensures that arrangements for equipment necessary in the home setting are made available.
• Attends to patient and family at time of patient’s death.
