Position Summary: The Hospice RN Case Manager is responsible for coordinating, planning, implementing and documenting care for hospice patients from admission through discharge. Utilizing physician orders, clinical judgment and interdisciplinary collaboration, the RN Case Manager develops and manages an individualized Plan of Care that meets each patient’s unique needs while ensuring compliance with organizational standards, state and federal regulations and Medicare Hospice Conditions of Participation. The RN Case Manager serves as a primary clinical contact for patients and families, providing skilled nursing care, symptom management, education and emotional support throughout the hospice journey.
