About The Position

At Iowa Home Care, we believe healing happens best at home. We are seeking a compassionate and skilled Registered Nurse (RN) to join our team as a Part-Time Home Health Wound Care Nurse. This position is ideal for a nurse who is passionate about wound management, patient education, and helping individuals achieve the best possible outcomes in the comfort of their own homes. As a Home Health Wound Care Nurse, you will provide specialized wound assessments, treatments, and ongoing clinical oversight while collaborating with physicians, patients, families, and the interdisciplinary care team. This position offers a flexible schedule and the opportunity to build meaningful relationships with patients while making a direct impact on their healing and quality of life.

Requirements

  • Current Registered Nurse (RN) license in the State of Iowa.
  • Minimum one year of nursing experience required.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Current CPR certification.

Nice To Haves

  • Home health and/or wound care experience preferred.
  • Strong assessment, critical thinking, and clinical documentation skills.
  • Ability to work independently and manage a flexible schedule.

Responsibilities

  • Perform comprehensive nursing assessments and develop individualized plans of care.
  • Assess, monitor, and treat a variety of wounds, including surgical wounds, pressure injuries, diabetic ulcers, vascular wounds, and traumatic injuries.
  • Perform wound measurements, staging, documentation, and ongoing evaluation of healing progress.
  • Administer wound care treatments in accordance with physician orders and evidence-based practices.
  • Educate patients, families, and caregivers regarding wound care, prevention strategies, nutrition, and disease management.
  • Monitor for signs and symptoms of infection or complications and communicate changes in condition to the physician and care team.
  • Coordinate care with physicians, therapists, aides, and other healthcare professionals.
  • Complete Start of Care, Recertification, Resumption of Care, and Discharge assessments as assigned.
  • Ensure accurate, timely, and compliant documentation within the electronic medical record.
  • Participate in case conferences and interdisciplinary team meetings.
  • Maintain compliance with all federal, state, and agency regulations.
