Home Health Visits SOC/Admissions Registered Nurse
About The Position
BAYADA Home Health Care is currently seeking a Full-Time Admissions Registered Nurse (RN) to join our home health visit team. You will complete OASIS Starts of Care (SOC) for adult and geriatric patients throughout southern Rhode Island. One year prior clinical experience as a licensed RN is required and Home Health and OASIS experience is strongly preferred. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. Requirements
- A current nursing license in good standing in the state of Rhode Island
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings).
Nice To Haves
- Home Health and OASIS experience is strongly preferred.
- Medicare Certified Home Health experience and OASIS experience is strongly preferred.
Responsibilities
- Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition
- Utilize your experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care)
- Triage and manage urgent clinical situations independently in the home setting
- Perform medication reconciliation, teaching, and safety monitoring
- Perform comprehensive wound assessment, care, and documentation
