About The Position

The Home Health Supervisor assists with essential department management, clinical oversight, and limited direct patient care functions to support safe, compliant, and high-quality home health services. This role provides first line supervision of clinical and non-clinical staff, supports operational coordination, leads process improvement efforts, and ensures adherence to regulatory and standard work requirements. This position may be filled by a licensed Registered Nurse (RN) or Physical Therapist (PT). In addition to supervisory responsibilities, the role may provide direct patient care as needed based on operational demands, patient needs, and licensure. Clinical decision-making authority is exercised in accordance with the incumbent's professional licensure and applicable regulations.

Requirements

  • a minimum of 2 years recent RN or PT experience
  • Bachelors in Nursing or Physical Therapy
  • 1 year, RN or PT skilled Medicare-certified home health experience including Start of Care OASIS assessment competence
  • 2 years, RN or PT experience
  • Registered Nurse (RN) - License, Active Arizona and/or Compact State licensure Required or Physical Therapist - License, Active Arizona and/or Compact State licensure Required
  • Basic Life Support (BLS) - Certification, Current CPR/BLS certification Required
  • Drivers License (DL) - License, Required
  • Auto Insurance - Certification, Required
  • Fingerprint Clearance Card (FPC) - Certificate, Fingerprint Clearance Card Level 1 Required
  • Verification the employee is not on the adult protective services registry Required

Responsibilities

  • Oversees coordination, implementation, and ongoing evaluation of patient plans of care delivered by interdisciplinary home health staff.
  • Ensures patient needs are continually assessed and services are delivered in accordance with quality, safety, and regulatory standards.
  • Collaborate with physicians and interdisciplinary partners to support effective, patient centered care delivery.
  • Oversee a clinical team within home health and supports department operations including scheduling, workflow coordination, productivity monitoring, and documentation compliance.
  • Participates in and leads process improvement initiatives; evaluates compliance to standard work and supports corrective actions as needed.
  • Facilitates effective use of technology and systems to support regulatory compliance, quality reporting, and billing requirements.
  • Supervises, coaches, and evaluates assigned clinical and non clinical staff.
  • Supports hiring, onboarding, competency validation, and ongoing staff development.
  • Provides guidance, mentoring, and performance feedback to promote accountability, engagement, and quality outcomes.
  • Participates in quality improvement and performance monitoring activities.
  • Ensures documentation accuracy and compliance with CMS, state, payer, and HonorHealth requirements.
  • Promotes a culture of safety, ethical practice, collaboration, and continuous improvement.
  • Provides first line supervision and operational decision making within defined departmental guidelines.
  • Clinical decision making authority is exercised within the scope of the incumbent’s professional licensure (RN or PT).
  • Functions restricted to RN licensure are performed by or in collaboration with appropriately licensed RN leadership when required.
  • Provides direct patient care on a limited and as needed basis, consistent with professional licensure, to support patient access, continuity of care, staffing needs, or complex clinical situations.
  • Performs assessments, interventions, treatments, and patient education within scope of licensure and organizational policy.
  • Documents care accurately and in compliance with regulatory and payer requirements.
  • Rotates after hours administrative call
