Home Health Clinical Supervisor (RN) Full-Time | Braintree, MA Are you an experienced Home Health RN who loves developing people, improving patient care, and keeping a clinical team moving forward? Requirements

  • Current, unrestricted Massachusetts RN license
  • Associate degree in nursing
  • At least two years of RN experience in home health or hospice
  • Strong OASIS, care coordination, and clinical documentation knowledge
  • A natural ability to coach, communicate, and bring out the best in others
  • Strong organization, attention to detail, and clinical judgment
  • Ability to learn systems such as Homecare Homebase and Workday
  • Dependable transportation, a valid driver’s license, and current auto insurance
  • Willingness to travel within the branch’s coverage area

Nice To Haves

  • Supervisory or clinical leadership experience preferred

Responsibilities

  • Overseeing patient care, clinical staffing, referrals, and assignments
  • Ensuring timely starts of care and seamless care coordination
  • Reviewing plans of care, physician orders, and clinical documentation
  • Conducting OASIS reviews to support accuracy, quality, and compliance
  • Monitoring patient needs and ensuring care plans remain appropriate
  • Coaching clinicians and providing ongoing education and feedback
  • Supporting recruiting, interviewing, onboarding, and staff development
  • Partnering with physicians, patients, families, providers, and vendors
  • Monitoring key clinical and operational performance metrics
  • Providing field support when needed
