About The Position

VNA of Ohio is seeking a detail-oriented, experienced RN to join their Home Health team in a dedicated Start of Care role. This position offers competitive pay and the necessary tools and support for effective patient care. The role involves being the first point of contact for patients, setting the tone for their home health experience. VNA of Ohio is a well-established home health agency with a long history, CHAP accreditation, and a 4-star rating from CMS.

Requirements

  • Active RN license (Ohio) + current BLS certification
  • Associate or Bachelor's degree in Nursing (BSN preferred)
  • Minimum 2 years of clinical nursing experience, including at least 1 year in a home health setting
  • Demonstrated OASIS experience and strong assessment skills
  • Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
  • Familiarity with Medicare and Medicaid regulations

Nice To Haves

  • OASIS-D certification or equivalent
  • Familiarity with HCHB or similar home health EMR
  • Experience in a dedicated SOC or admissions nursing role
  • Strong time management skills and ability to manage a high-volume assessment caseload

Responsibilities

  • Conduct comprehensive Start of Care (SOC) and Resumption of Care (ROC) assessments for new and returning home health patients.
  • Complete accurate and thorough OASIS assessments in compliance with Medicare, Medicaid, and agency standards.
  • Develop individualized, evidence-based care plans in collaboration with the referring physician and interdisciplinary team.
  • Establish rapport with patients and families from day one, setting clear expectations for the home health episode of care.
  • Identify patient goals, barriers to care, and safety concerns during the initial visit.
  • Coordinate with RN Case Managers and clinical supervisors to ensure seamless handoff and continuity of care following SOC.
  • Maintain accurate and timely documentation in the EMR within required timeframes.
  • Participate in team meetings, case conferences, and ongoing professional development.
