About The Position

Human Touch Home Health is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our growing team serving patients throughout Richmond, Henrico, Chesterfield, Mechanicsville, Glen Allen, Midlothian, Hanover, and surrounding communities. As a Home Health Speech-Language Pathologist, you'll help patients overcome challenges related to speech, language, cognition, communication, and swallowing while improving their independence and quality of life in the comfort of their own homes. Requirements

Active Virginia Speech-Language Pathologist License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment and Documentation Skills
Excellent Communication and Interpersonal Skills

Nice To Haves

Certificate of Clinical Competence (CCC-SLP) Preferred
Home Health Experience Preferred but Not Required
Experience with OASIS is a plus

Responsibilities

Complete speech, language, cognitive, and swallowing evaluations
Develop individualized treatment plans based on patient goals and clinical needs
Provide skilled speech therapy services in patients' homes
Educate patients and caregivers on communication techniques and swallowing strategies
Monitor patient progress and adjust treatment plans as needed
Collaborate with physicians and interdisciplinary team members
Complete timely and accurate clinical documentation
