About The Position

Provides speech therapy services to Home Health patients requiring such services. Operates independently under the supervision of supervisor or manager. The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned unit. The individual must demonstrate knowledge of the principles of growth and development across the life span and assess data reflective of the patient's requirements relative to his/her age-specific needs and to provide the care needed as described in the unit's departmental policies and procedures. Communicates with and documents communications with the attending physicians, appropriate other disciplines (RNs, CNAs, OTs, STs MSWs) and case managers regarding patient condition, discharge planning, plan of care, Patient/family problems, and third party payer issues. Provides verbal and written instruction to patient, family, nursing and home health aides. Manages caseload independently and efficiently. Provides home care support for patients and family members via skilled visits, joint home visits with care team. Ensures that patient and family needs, related to the diagnosis and/or disability, are met through the care plan in collaboration with the attending physicians and other disciplines. Implements an individualized treatment plan through the use of skilled, evidence-based therapy techniques, addressing disorders related to speech, language, cognitive-communication, voice, swallowing and fluency. Provides verbal and written instructions to patients, caregivers, and family members as needed. Develops, implements, and oversees the "speech therapy only" care plan, collaborating with the health care team to achieve and formulate a cohesive, comprehensive plan of care. Monitors provision of care to ensure appropriate outcomes, visit frequency and compliance with documentation required. Participates in ongoing quality assurance and improvement programs, attends meetings, evaluates outcomes, and conducts audits.

Requirements

  • Bachelors in in Speech Language from an accredited university
  • 1 year in related
  • Speech-language Pathologist (SLP) - License, Speech Language Practitioner Required
  • Drivers License (DL) - License, Driver License Required
  • Basic Life Support (BLS) - Certification, Required
  • Fingerprint Clearance Card (FPC) - Certificate, State Current Arizona Dept. of Health Services Fingerprint Clearance Card or shall apply and be eligible for a fingerprint clearance card within twenty (20) working days of employment Required
  • Current automobile insurance Required
