About The Position

We're seeking a compassionate and skilled Speech Language Pathologist (SLP) to join our team and make a meaningful difference in the lives of patients recovering from illness, injury, or managing chronic conditions. As a Home Health SLP, you'll evaluate, develop, and implement individualized treatment plans that improve communication, cognition, and swallowing while helping patients maintain their independence and quality of life.

Requirements

  • Current Speech Language Pathologist (SLP) license in the applicable state
  • Current CPR certification
  • Valid driver's license
  • Reliable transportation with current auto insurance

  • Minimum of one year of rehabilitation or speech therapy experience
  • Home Health experience
  • Long-Term Care experience
  • HomeCare HomeBase (HCHB) experience
  • VitalStim Certification

Responsibilities

  • Evaluate patients with speech, language, cognitive, voice, and swallowing disorders.
  • Develop personalized treatment plans based on comprehensive clinical assessments.
  • Treat patients with dysphagia, aphasia, cognitive impairments, voice disorders, fluency disorders, and other communication challenges.
  • Perform swallowing assessments and recommend therapeutic exercises, dietary modifications, and compensatory strategies.
  • Educate patients and caregivers on communication techniques, home exercise programs, and safe swallowing practices.
  • Recommend and train patients on alternative or augmentative communication systems when appropriate.
  • Collaborate with physicians, nurses, therapists, and other interdisciplinary team members to ensure coordinated, high-quality patient care.
  • Document evaluations, treatment plans, patient progress, and outcomes accurately and in compliance with agency and regulatory standards.
  • Participate in case conferences and interdisciplinary meetings to support exceptional patient outcomes.
