Franciscan VNS Home Health And Hospice is seeking a dedicated Home Health Speech Language Pathologist. Good speech therapists help patients regain communication, cognition, and swallowing skills. Requirements

  • Master's Degree- Speech and Language Therapy- Required
  • 1 Year Speech Therapy Experience - Required
  • Speech Pathologist (SPE)- State Licensing Board - Required
  • American Speech Language and Hearing Association (ASHA) -Required
  • Cardiopulmonary Resuscitation (CPR)- American Heart Association - Required
  • Driver's License - Required
  • Auto Insurance - Required

Nice To Haves

  • 1 Year Home Health Experience - Preferred

Responsibilities

  • Perform comprehensive speech and language evaluations, treatment and related follow-up services, including discharge planning.
  • Document patient assessments and pertinent information in a timely manner.
  • Complete appropriate goal re-assessment and discharge planning.
  • Provide speech therapy, including selection, and use of assistive equipment.
  • Collaborate with patient and other healthcare personnel, or outside agencies, regarding coordination of care.
  • Provide speech and language therapy, according to standards of care defined by the state practice act, and other professional and governing agencies.
  • Train and educate patients and family members or others support, and document understanding of treatment plan and goals.
