About The Position

Classic Home Care, a nurse-owned and operated skilled home health agency, is seeking a motivated and compassionate Speech Language Pathologist (SLP) to join their team as a home health / home care SLP. This contingent position is perfect for a quick learner who thrives in a collaborative, team-oriented environment. Classic Home Care has been serving communities across Southeast Michigan since 2003 and is Joint Commission-accredited and Medicare-certified. The agency is led by registered nurses (RNs) who actively collaborate with the dedicated team, fostering a supportive and professional environment.

Requirements

Active Michigan Speech Therapy license
Reliable transportation
Valid driver's license and auto insurance in your name as a driver

Home health care experience preferred

Responsibilities

Provides services in speech pathology or audiology in accordance with the physician’s plan of treatment.
Accepts only patients referred by the patient care manager or designee.
Demonstrates competency in the skills required by the agency on at least a biannual basis.
Assists the physician in assessing the patient’s level of functioning and the development (and revision, as necessary) of the plan of care by applying appropriate tests for speech, hearing and language disorders.
Sets appropriate goals for the patient.
Records and reports the patient’s reaction to treatment and any changes in the patient’s condition to the physician and/or patient care manager and case manager.
Instructs and advises the patient, family and other health team personnel in the phases of speech pathology in which they may assist the patient. The goal is to teach and promote disease prevention for eventual patient & caregiver independence, utilizing agency materials as appropriate.
Recommends hearing tests by audiologist when necessary.
Checks hearing aides.
