About The Position

Provides medical social services to patients and families having emotional, social, environmental, and financial problems under the clinical supervision and oversight of an MSW from a school of social work accredited by the CSWE. Tower Health at Home provides a comprehensive range of services that allows patients to recover from a short-term illness or manage a chronic condition in the comfort of their own home. Our team of healthcare professionals work collaboratively to deliver compassionate, high-quality care in our patient’s home for their home health or hospice needs.
