About The Position

BioMechanics is a PT-owned and operated company with over 10 years of dedicated service in delivering exceptional home health care. We are seeking dynamic Social Workers to join our team. Our mission is to empower patients through compassionate care, advocacy, and support. This role involves evaluating and supporting patients in their homes, focusing on emotional well-being, family dynamics, discharge planning, and resource coordination. The position also includes providing support to patients and their families in navigating healthcare systems and connecting with community resources, documenting progress with easy-to-use laptop software, and managing a caseload of 1 to 7 patients per week.

Requirements

• Licensed Social Worker in California (LCSW or MSW preferred).
• CPR Certification.
• Recent physical exam within the past year.
• Clear TB Test.
• Strong communication and interpersonal skills.
• Ability to work independently and manage a flexible caseload.

Responsibilities

• Evaluate and support patients in their homes, focusing on emotional well-being, family dynamics, discharge planning, and resource coordination.
• Provide support to patients and their families in navigating healthcare systems and connecting with community resources.
• Document progress with easy-to-use laptop software.
• Manage a caseload of 1 to 7 patients per week, ensuring each patient receives compassionate and personalized care.
