About The Position

The Home Health Scheduler (PSC) is responsible for scheduling patient services and coordinating care with agency staff. This position also cross-trains and provides coverage as a Medical Records Specialist (MRS).

Requirements

  • High school diploma or equivalent required.
  • Valid, current driver’s license required.
  • Reliable transportation and current automobile liability insurance required.
  • Ability to establish and maintain effective working relationships with employees across all areas of the agency.
  • Strong interpersonal, communication, organization, and scheduling skills.
  • Ability to manage competing priorities and respond effectively to changing patient and staffing needs.

Nice To Haves

  • Graduation from an accredited vocational nursing program preferred.
  • Current state license as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) required. A multistate license issued by a member state of the Nurse Licensure Compact (NLC) is preferred.
  • Current CPR certification required if assigned to make patient visits.
  • At least one year of home care experience, including home health or hospice, preferred.
  • Working knowledge of electronic medical record systems preferred.
  • If clinically licensed, nursing skills consistent with generally accepted professional standards of practice.

Responsibilities

  • Complete workflow tasks and other assignments directed to the Patient Services Coordinator’s responsible position within the agency’s electronic medical record system.
  • Prioritize and schedule new patient admissions in coordination with the Administrator or Case Manager–Registered Nurse.
  • Coordinate patient care with nurses, patients, caregivers, and other members of the interdisciplinary team.
  • Communicate effectively and professionally with patients, staff, healthcare professionals, and referral sources.
  • Respond to agency and patient needs with professionalism, creativity, and sound judgment.
  • Demonstrate an ongoing commitment to professional growth and competency.
  • Participate in agency-sponsored in-service training.
  • Participate in the agency’s Quality Assessment and Performance Improvement (QAPI) program as directed.
  • Promote and follow the agency’s philosophy, policies, and procedures to support high-quality patient care.
  • Cross-train and provide support as a Medical Records Specialist as needed.
  • If clinically licensed, perform assigned patient visits and complete all required documentation promptly.
