This position will work out of our South Charleston Agency and will assist the organization in achieving a centralized, efficient scheduling protocol of its multi-channel Home Services resources that will optimize patient care, resource optimization and coordination of service offerings. Requirements

  • Multi-task and prioritize and effectively
  • Schedulers should be able to talk with healthcare workers, doctors, nurses, families and patients on a daily basis. They must be friendly and be able to listen to people’s needs and concerns and explain complicated concepts in a clear and concise way.
  • Effectively communicate with the patient and their families. They will need to understand their needs and concerns.
  • Be efficient working with medical records; must be organized and able to process the information
  • Proficient with computers to document patient data
  • High School degree and/or related Associate Degree preferred
  • Current LPN license preferred
  • Microsoft Office proficient
  • Ability to relate with people on the telephone, in a pleasant, business-like manner.
  • Detail oriented.
  • Familiar with computer operations.
  • Knowledge of medical terminology desirable.
  • Ability to establish and maintain effective working relationships with patients, employees and the public.

Responsibilities

  • Responsible for meeting all service standards for exceptional customer service delivery.
  • Responsible for telephone and in-person customer service support
  • Coordinate the processing of clinical documentation.
  • Confirm accuracy and enter time and travel in database.
  • Schedule patient encounters in database.
  • Monitor compliance with home health aide supervision visits and functional reassessments.
  • Operate a variety of standard office machines, including a personal computer and a variety of computer software, phone, fax, calculator, shredding machine and photocopy machine.
  • Use personal judgment and initiative to develop effective and constructive solutions to challenges and obstacles in referral activity and procedures.
  • Evaluates applicable logistic software and POC applications to maximize logistics scheduling for the organization
  • Maintain personal ability in, and appropriate use of, all relevant ICT (Information and Communications Technology) and other systems within the office management function
  • Establish and maintain effective working relationships with co-workers, supervisors and the general public.
  • Fosters an atmosphere of teamwork and collaboration.
  • Is consistently supportive of the agency administration and missions and is a positive team member at all times.
  • Engages in other job tasks as assigned by Director of Operations.
