Requirements
- Current, unrestricted Massachusetts RN license
- At least one year of clinical nursing experience
- Strong assessment, communication, and time management skills
- Ability to work independently and travel throughout Springfield
- Valid driver’s license, current auto insurance, and reliable transportation
Responsibilities
- Providing skilled nursing visits
- Developing and updating care plans
- Managing an assigned caseload
- Coordinating with physicians and other clinicians
- Teaching patients and caregivers how to manage medications, treatments, and health conditions at home
