Requirements

  • Active RN license in the State of Pennsylvania
  • CPR certification
  • At least 1 year of experience as a registered nurse
  • Strong organizational and communication skills
  • Reliable transportation, a valid driver's license, and insurance
  • Availability for a very minimal on-call rotation, including weekends, evenings, and holidays

  • Prior home health experience preferred

Responsibilities

  • Provide admission, care management, and follow-up visits utilizing your nursing process: assess, diagnose, plan, implement, evaluate.
  • Utilize our brand-new Artificial Intelligence system to help keep documentation minimal and patient care your focus.
  • Build the plan of care for new patients and keep it current with our interdisciplinary team.
  • Deliver hands-on nursing care, including physical assessment, medication management and administration, wound care, IV therapy, tube feeding, durable medical equipment setup and training, patient education, and comfort care.
  • Coordinate care with physicians, PAs, nurse practitioners, home health aides, physical and occupational therapists, and other clinicians.
  • Travel to patients in their homes or facilities.
  • Keep each patient's physician updated on care needs and progress.
  • Supervise LPNs, home health aides, and personal home care aides as needed.
  • Teach patients and families the home care procedures and safety strategies that keep patients safe at home.
  • Maintain full compliance with HIPAA and all state and federal regulations.
  • Document clinical notes clearly and concisely.
