About The Position

Amedisys is hiring a registered nurse case manager for its Suffolk Territory. The role is full-time, day shift, with a weekend call rotation of one weekend per month. The nurse helps patients heal at home by assessing their needs and coordinating care with the care team. Compensation is $82,000–$88,000 annually, with point pay after orientation and a $7,500 bonus.

Requirements

• At least one year of clinical experience as a Registered Nurse (RN); candidates with less than one year must be approved by the VP Clinical.
• Current RN license for the state or states assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Assess patients and collaborate with the care team to develop and implement plans of care.
• Refer patients to other disciplines as indicated by their needs, or document the rationale for not doing so.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
