About The Position
Phoenix Home Care & Hospice is a mission-driven company dedicated to providing exceptional care to our clients. This role involves serving Kansas City, MO and Surrounding Communities with a unique 7 Days On / 7 Days Off schedule, including on-call responsibilities for overnights and weekends.
Requirements
- Active RN Nursing License in the state.
- Ability to work in a fast-paced, dynamic environment.
- Excellent organizational and self-management skills.
- Strong and compassionate customer service skills.
- Valid driver’s license and reliable transportation.
Responsibilities
- Skilled Visits to patients based on their needs.
- Daily patient interaction, ensuring comfort and support.
- Education on health conditions including, but not limited to, CHF, COPD, Diabetes.
- Wound care and assessments.
- Medication management and administration.
- Change dressings, bandages, and other contraptions as prescribed.
- Documentation of patient’s condition, reporting any significant changes to the physician.
- Collaboration with other healthcare professionals, including Physical, Speech, and Occupational Therapists.
