About The Position

Residential Home Health and Hospice is hiring a Registered Nurse to provide skilled nursing care to patients in their homes in the Carterville area. The role offers autonomy, meaningful patient relationships, and opportunities for career growth. The nurse works independently while collaborating with an interdisciplinary team and receives support from a dedicated field education team and leadership. The position is field-based and requires travel to patient homes within an assigned territory.

Requirements

Active Registered Nurse (RN) license in the state where the employee works.
One year of RN experience and the clinical expertise to deliver quality patient care.
Valid driver’s license.
Ability to spend approximately 20% of the workday driving to and from patient locations.
Successful completion of a drug screen prior to employment.

Responsibilities

Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team.
Perform patient assessments, medication management, and care planning.
Educate patients and families about disease management and treatment plans.
Use critical thinking and problem-solving skills in real time.
Complete thorough, compliant EMR documentation.
Travel to patient homes within the assigned territory.
