About The Position

Guthrie Home Health is seeking a Per Diem Home Health RN based out of Waverly, NY, supporting patients across the Southern Tier region near Elmira and Binghamton. This role is ideal for an RN who values flexibility, independent clinical judgment, meaningful one-on-one patient relationships, and the support of an experienced Home Health team. In this per diem role, you will organize your day around your patient load, provide skilled nursing care in the home setting, and help patients regain confidence, safety, and independence. Requirements

  • Registered Nurse currently licensed in the state of practice.
  • At least one year of experience as a Registered Nurse.
  • OR less than one year of experience if transitioned from the Newly Licensed Registered Nurse Residency (NLRN) Program.
  • BLS certification.
  • Proficiency in Information Technology, such as electronic health records, communication systems, computers, and equipment necessary to perform essential functions of the position.
  • Demonstrated interpersonal skills that convey a positive and supportive attitude.

Nice To Haves

  • BSN in Nursing preferred.
  • Specialty Certification is preferred.
  • ACLS is unit dependent.
  • Experience in independent clinical decision-making, patient education, and coordinating care in the home setting.
  • Experience with time management and documentation.
  • Experience working as part of an interdisciplinary care team.
  • Interest in applying strong assessment skills in a more autonomous community-based setting.
  • Experience in a hospital, clinic, long-term care, or home health RN role.

Responsibilities

  • Provide skilled nursing care in the home setting.
  • Help patients regain confidence, safety, and independence.
  • Organize your day around your patient load.
  • Assess, diagnose, plan, implement, and evaluate patients' care.
  • Develop, update, and coordinate the patients' plan of care to achieve patient goals and to optimize outcomes and transitions across the continuum.
  • Monitor, record, and communicate patient condition as appropriate.
  • Perform and maintain the currency of essential competencies as required by a specific area of hire and population served.
  • Effectively advocate for ethical and holistic care by partnering in care planning to promote the autonomy, dignity, rights, values, and beliefs of those served.
  • Foster a learning environment for patients/families, nursing, and other members of the healthcare team, including students.
  • Facilitate formal and informal learning for patients/families, nursing, other members of the healthcare team, and the community.
  • Accountable for safety, identifies and corrects problems, and integrates evidence and best practice into nursing care; uses data and evidence to improve patient outcomes.
  • Align patient safety, self-safety, and environmental safety as equally important for best outcomes.
  • Engage in ongoing professional development; practice at the top of license within the legal parameters of the Nurse Practice Act, the ANA Code of Ethics for Nurses, and specialty standards.
  • Promote a culture of inquiry that explores, integrates, and disseminates research and evidence-based practice.
  • Demonstrate leadership as change agents using the collaborative leadership skills of advocacy, influence and innovation.
  • Partner with colleagues, appropriately delegate, coordinate care, and collaborate with others as equal members of the inter-professional care team to integrate nursing knowledge.
  • Model professional behaviors as a representative of the nursing profession.
  • Participate in the planning and presentation of educational programs.
  • Work with a wide range of staff as part of an interdisciplinary team, including physicians, nurses, and ancillary staff.
  • Use independent, critical judgment in all aspects of patient care delivery.
  • Effectively manage multiple responsibilities, urgent responses, and challenging situations.
