This is a full time position that will service the Central and Northern Tucson areas (specifically 85741, 85704, and 85705). Annual salary range $85000 - $95000+ per year (depending on experience), plus travel expenses.
Requirements
- Currently licensed as a Registered Nurse (RN) in the State of Arizona
- Home Health experience a plus
- Strong clinical skills and a passion for patient-centered care
- Expert organizational and time management skills
- Current CPR certification
- Valid driver’s license, auto insurance, and reliable transportation
Responsibilities
- Build meaningful relationships with patients and families
- Create personalized care plans to improve patient outcomes and support their independence
- Coordinate care as a trusted Case Manager
- Educate and empower patients and caregivers with confidence
- Collaborate with an interdisciplinary team that shares your commitment to excellence
