About The Position

Boost Home Healthcare makes it easier for patients to focus on recovery and wellness by personalizing and coordinating care. As a member of our team, you’ll play a key role in providing quality home healthcare, where patients are able to remain independent at their place of residence in the healthiest and happiest state possible. The Primary Care Registered Nurse (Primary Care RN) is the agency employee assigned primary responsibility for planning and implementing the patient's care. The Primary Care RN provides skilled nursing care to patients on an intermittent basis in their place of residence, in accordance with physician orders and the patient's plan of care, under the direction of the Supervisor of Clinical Services. The Primary Care RN also serves as the supervising nurse for homemaker-home health aides assigned to their patients.

Requirements

  • Graduate of an accredited school of professional nursing
  • Active, unrestricted license to practice as a Registered Nurse in the State of Connecticut
  • Minimum of two (2) years of nursing experience, with at least one (1) year in home health care or a community health program that included care of the sick at home
  • Current BLS certification
  • Valid Connecticut driver's license, reliable transportation, and current auto insurance
  • Ability to travel locally throughout the agency's service area
  • Strong clinical assessment, documentation, and communication skills
  • Passion for serving patients and families with compassion and professionalism

Responsibilities

  • Conduct initial home assessments and admit patients to service
  • Develop, implement, and revise individualized patient care plans
  • Provide skilled nursing care requiring specialized clinical judgment
  • Coordinate care with patients, families, physicians, and the interdisciplinary team
  • Evaluate patient progress and adjust the plan of care as needed
  • Identify needs for additional services and make appropriate referrals
  • Develop written assignments for home health aides and supervise them in the home, including supervisory visits at least every four weeks
  • Prepare timely, accurate clinical notes and progress documentation
  • Participate in case management conferences, in-service education, and the agency's quality assurance program
  • Ensure compliance with all federal, state, and agency policies, including HIPAA and the Patient's Bill of Rights
