Requirements

Graduate of an accredited school of nursing and current RN License in SC or compact license.
1 year of experience in home health, preferred.
Proficient computers skills and workplace software knowledge.
Work with people, function well under stress, use clinical judgment, be creative and accept challenges.
Able to effectively deal with multiple changes based on patient needs and scheduling.
Clinical proficiency in nursing, strong communication skills, and compassion.
CPR Certification
Valid driver's license and proof of insurance

Responsibilities

Provide patient care after-hours as needs arise for our patients
Enable patients to spend quality time with loved ones and participating
Work as part of an interdisciplinary team comprised of a physician, LPN, CNA, HHA, social worker, and volunteers to carry out the patient’s plan of care.
Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments.
Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies
Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals.
Take a holistic approach to care that engages the patient’s mind, body, spirit and family.
