About The Position

We are known for clinician support, patient-first care, and long-term stability. We are hiring an experienced Home Health RN to join our evening team serving the Phoenix metro area. This role offers manageable caseloads, strong clinical support, and a predictable schedule — without hospital demands or overnight shifts.

Requirements

Active Arizona RN license (Compact RN license accepted)
Home health experience required (2+ years of direct patient care strongly preferred)
Graduate of an accredited nursing program (ADN or BSN accepted)
Current CPR/BLS/First Aid certification
Valid Arizona Level One Fingerprint Clearance Card (standard for home health in AZ)
Valid AZ driver's license, reliable transportation, and proof of auto insurance
Current TB test results

Responsibilities

Perform skilled nursing visits (wound care, wound vacs, IV therapy, medication administration)
Conducting Start of Care (SOC), Resumption of Care (ROC), recertification, and discharge assessments
Manage central lines (PICC, ports) and perform venipuncture
Develop and update Plans of Care in collaboration with physicians
Educate patients and caregivers on treatments, medications, and safety
Coordinate care with interdisciplinary teams (LPNs, CNAs, therapy, pharmacy)
Complete EMR documentation and support discharge planning
