About The Position
Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. We are hiring a Registered Nurse (RN) for a Home Health position providing skilled nursing care to patients in their homes in the Bad Axe area. This role is eligible for a $10,000 first year success bonus, with bonus opportunities extending beyond the first year. Requirements
- Active Registered Nurse, RN license in the state you work.
- One year of RN experience and the clinical expertise to deliver quality patient care.
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
- Legally authorized to work in the United States.
Responsibilities
- Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team.
- Perform patient assessments, medication management, and care planning.
- Educate and empower patients and families on disease management and treatment plans.
- Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
- Work independently, but never alone.
- Travel to patient homes within assigned territory.
