About The Position

This role involves collaborating in establishing the home health plan of care, coordinating and implementing care delivery, and anticipating patient care needs across the continuum. The nurse will analyze comprehensive information pertinent to the patient's care and management to determine nursing diagnosis. Continuous utilization of the nursing process is expected to evaluate patient's progress towards goals and apply appropriate interventions for effective outcomes. Strategies to promote health and a safe and healthy environment will be employed. The responsibilities listed are a general overview, and additional duties may be assigned.

Requirements

LIC-Registered Nurse - Licensure-Others
Relevant Work Experience
1 year experience
Graduate of an approved discipline specific program
