About The Position

This is a Full Time Case Manager position within the Home Health department. The role involves developing and maintaining therapeutic relationships, providing individualized care, and collaborating with patients, families, and healthcare teams to ensure quality, cost-effective care. The position requires assessing patient needs, implementing care plans, and participating in quality improvement initiatives. Education and professional development are key components, as is the application of the nursing process and ethical decision-making. The role also involves leadership, collaboration, and continuous learning to enhance personal and departmental competence.

Requirements

  • Graduate from an accredited school of nursing
  • Earn 10 continuing education credits annually
  • One year of medical surgical experience
  • Has as demonstrated proficiency for the requirements for RN role
  • Good communication and interpersonal skills
  • Good documentation skills.
  • Computer skills
  • Good time management and organizational skills
  • Familiarity with patient care equipment
  • Current Illinois RN license
  • Current CPR

  • Demonstrates ability to critique own nursing practice and achieves required certifications or Bachelor’s Degree.
  • Earns a minimum of 10 contact hours per year.
