Requirements
- Active Iowa RN license
- Strong communication, empathy, and organizational skills
- Valid driver's license and reliable vehicle for local Iowa travel
Nice To Haves
- Home health or case management experience preferred
Responsibilities
- Help patients stay stable, independent, and safely at home.
- Perform 60-day visits to re-certify the patient's need for care.
- Follow up with the physician on a patient's status.
- Conduct bi-weekly skilled visits to manage medications or a chronic condition.
- Conduct thorough assessments and provide proactive care.
- Complete documentation and collaborate with the team.
