The Home Health RN Case Manager will develop and maintain a therapeutic relationship throughout the healthcare continuum, demonstrating adaptive communication styles based on individual patient needs. This role provides individualized care to promote physical and psychosocial comfort, assesses patient and family readiness, and involves them in planning and implementing care. The position also involves mobilizing appropriate resources, adapting practice to the latest standards, participating in quality improvement initiatives, and meeting regulatory requirements. Additionally, the RN Case Manager will educate patients and families, assist in creating an environment for educational growth for peers, and utilize the nursing process as a problem-solving model. This includes recognizing changes in clinical situations, evaluating and implementing changes to the plan of care, making sound clinical decisions for complex patient problems, prioritizing and organizing patient care, initiating referrals and discharge planning, applying ethical decision-making, monitoring for confidentiality, and delegating correctly to unlicensed assistive personnel. The role also involves demonstrating leadership behaviors, participating in unit meetings, maintaining current knowledge, and enhancing communication, feedback, and conflict resolution skills.
