About The Position

The Home Health RN Case Manager will develop and maintain a therapeutic relationship throughout the healthcare continuum. This role involves providing individualized care to promote physical and psychosocial comfort, assessing patient and family readiness, and involving them in planning and implementing care. The Case Manager will mobilize appropriate resources, adapt practice to the latest standards, and participate in quality improvement initiatives. Additionally, the role includes patient and staff education, demonstrating the use of the nursing process, and applying ethical decision-making. Leadership and collaboration are key, requiring respect and understanding for peers and other clinical disciplines, and participation as an effective member of the patient care team. The position requires commitment to the development of self and others, and participation in unit meetings and activities that improve patient care or administrative systems.

Requirements

  • Graduate from an accredited school of nursing.
  • Earn 10 continuing education credits annually.
  • One year of medical surgical experience.
  • Has demonstrated proficiency for the requirements for NC I.
  • Good communication and interpersonal skills.
  • Good documentation skills.
  • Computer skills.
  • Good time management and organizational skills.
  • Familiarity with patient care equipment.
  • Current Illinois RN license.
  • Current CPR.

  • Demonstrates ability to critique own nursing practice and achieves required certifications or Bachelor’s Degree.
  • Earns a minimum of 10 contact hours per year.
