We are seeking a dedicated full-time Registered Nurse (RN) to join our Home Health team serving Arlington, Belmont, Watertown, Cambridge, Somerville, and surrounding communities. In this role, you will provide compassionate, one-on-one care in patients’ homes, supporting their independence and improving quality of life while building meaningful patient and family relationships.

Requirements

  • Current, unrestricted state RN license.
  • 1+ year experience in a clinical care setting as a nurse.
  • Valid driver’s license and insurance and reliable transportation to perform job tasks
  • Able and willing to travel within the branch/office coverage area.
  • Must be able and willing to travel 50%.
  • Ability to sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.

Responsibilities

  • Manage a caseload of patients in their homes, providing skilled nursing care, care coordination, patient education, and ongoing assessments.
  • Collaborate closely with physicians, caregivers, and interdisciplinary team members to develop and implement individualized plans of care that promote positive patient outcomes.
  • Conduct comprehensive patient assessments and develop individualized plans of care.
  • Perform initial evaluations and ongoing nursing visits based on patient needs and physician orders
  • Monitor patient conditions and identify physical, psychosocial, environmental, and caregiver-related factors that may impact health outcomes.
  • Coordinate care with physicians, therapists, social workers, and other members of the healthcare team.
  • Communicate significant changes in patient condition and recommend appropriate interventions.
  • Update and revise care plans as patient needs evolve.
  • Provide education and support to patients, families, and caregivers.
  • Maintain accurate, timely clinical documentation and ensure compliance with regulatory and company standards.
  • Protect patient confidentiality and adhere to all HIPAA requirements.
