Requirements
- Active, unrestricted RN license (Connecticut)
- At least 1 year of clinical nursing experience
- Ability to travel locally (you’ll spend time in the field—this is not a desk job)
- Valid driver’s license, reliable vehicle, and current auto insurance
- Ability to safely assist patients (lifting, positioning, etc.)
Nice To Haves
- Home Health or Hospice experience
- Familiarity with HomeCare HomeBase
- Associate’s or Bachelor’s Degree in Nursing
Responsibilities
- Manage a dynamic caseload of patients in your local community
- Perform comprehensive assessments and develop individualized care plans
- Provide hands-on skilled nursing care and patient education
- Monitor patient progress and adjust care plans as needed
- Collaborate with physicians, therapists, and interdisciplinary teams
- Be a trusted partner for patients and families throughout their care journey
