About The Position

The RN Case Manager is responsible for coordinating client care, acting as the primary point of contact for all disciplines involved in patient care, and overseeing visit frequency for the episode of care. This role involves consulting with physicians and the office regarding patient care, performing accurate OASIS collection, maintaining current medication profiles, ensuring timely reporting of lab values to physicians, and participating in weekly case conferences and monthly case manager meetings. The position requires providing patient care with respect for patient rights, dignity, and needs, assessing physical, psychological, and social needs, managing pain, and documenting patient information according to guidelines. The RN Case Manager must also review patient history, diagnostics, and lab data, reporting any abnormalities.

Requirements

  • Current, unencumbered RN or LPN license
  • Certification in CPR
  • Prior clinical experience
  • Excellent communication skills
  • Demonstrated ability to maintain a high level of professionalism during stressful times
  • Strong critical thinking skills
  • Ability to adapt to different clinical settings and environments

Responsibilities

  • Perform visits and complete coordination of client care.
  • Act as the point of contact for all disciplines involved in providing care to patients.
  • Oversee the frequency of visits for the episode of care.
  • Consult with the physician and the office, providing details about patient care.
  • Perform accurate OASIS collection.
  • Ensure the medication profile remains current.
  • Ensure lab values have been reported to the physician timely.
  • Attend weekly case conference and monthly case manager meetings.
  • Provide all patient care in a manner that reflects respect for patient rights, dignity, and expressed needs.
  • Assess the patient’s physical, psychological, and social needs.
  • Assess pain and implement appropriate pain management techniques.
  • Document patient history, symptoms, assessments, tests, and care according to guidelines.
  • Review patient history, diagnostics, and lab data, and report any abnormalities.
