About The Position

This is a full-time position for a Home Health RN Case Manager in the 34100 North Metro C - Nursing Department. The role requires availability Monday through Friday, 8 AM to 5 PM, with a requirement for weekend, on-call, and one summer/one winter holiday availability. The position serves Elgin and surrounding areas including Pingree Grove, Streamwood, Bartlett, and Hanover Park. The pay range is $38.20 - $57.30 per hour.

Requirements

  • Nurse, Registered (RN)
  • A valid driver’s license issued by the Division of Motor Vehicles.
  • Graduate from an accredited school of nursing
  • Earn 10 continuing education credits annually
  • One year of medical surgical experience
  • Has as demonstrated proficiency for the requirements for RN role
  • Good communication and interpersonal skills
  • Good documentation skills.
  • Computer skills
  • Good time management and organizational skills
  • Familiarity with patient care equipment
  • Current Illinois RN license
  • Current CPR
