About The Position
Immediate openings for Registered Nurse RN Case Managers in Home Health in Scranton, PA and surrounding areas. Manage your own caseload, work one-on-one with patients, and avoid hospital burnout.
Requirements
- Active Pennsylvania Registered Nurse RN license
- Minimum 1 year home health RN experience
- CPR certification
- Valid driver’s license and reliable transportation
Nice To Haves
- Case Manager or Case Management experience preferred
Responsibilities
- Manage a home health caseload
- Perform RN assessments and reassessments
- Develop and update care plans
- Coordinate care with physicians and healthcare team
- Complete home health documentation and ensure compliance
- Educate patients and families
