Requirements

  • Graduate of an accredited school of nursing.
  • One (1) to two (2) years of recent acute care experience in an institutional setting.
  • Current RN licensure and CPR certification.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

Responsibilities

  • Conduct comprehensive initial and ongoing assessments of patients and their families.
  • Develop, implement, and update individualized care plans based on physician orders and patient needs.
  • Identify therapeutic, preventive, and rehabilitative interventions that support positive patient outcomes.
  • Provide skilled nursing care in accordance with physician orders and the State Nurse Practice Act.
  • Administer medications, treatments, and clinical interventions as prescribed.
  • Monitor patient conditions and make appropriate recommendations for care plan adjustments.
  • Educate patients and caregivers on diagnoses, medications, treatments, and disease management.
  • Coordinate care across multiple disciplines to ensure seamless service delivery.
  • Collaborate with physicians, therapists, social workers, home health aides, and community providers.
  • Advocate for patient needs and help connect families with available resources.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Communicate patient status changes promptly to physicians and obtain necessary orders.
  • Provide regular updates to the care team to ensure continuity of care.
  • Empower patients and families through education and ongoing support.
  • Encourage caregiver involvement in the care planning process.
  • Promote independence, safety, and improved quality of life.
  • Instruct, supervise, and evaluate home health aides according to agency standards.
  • Conduct required supervisory visits and support quality care delivery.
  • Serve as a clinical leader and resource for the interdisciplinary team.
